Diabetes Care Program: Hazel's Story
After being diagnosed with diabetes in 2017, Hazel received little to no training to use her diabetes devices. She decided to come to the Stanford Diabetes Care Program, where her care team trained on her all the diabetes devices she used, and provided her with helpful tools and education to manage her condition and improve her quality of life.

Our doctors, advanced practice providers, nurses, pharmacists, nutritionists, and other specialists collaborate daily on the best ways to care for each patient. Many of our doctors and nurses are also Certified Diabetes Educators®. This certification means they specialize in treating diabetes. Our experienced team works together to develop a personalized plan for you, with the latest therapies to manage diabetes.

We make it easy to access the diabetes care and support you need. With four convenient locations in the Bay Area. You can attend our Diabetes Education & Prevention Classes and receive individual education in Palo Alto, Santa Clara and Emeryville. A fourth location in Menlo Park provides individual consultation and training.
