If you need to reschedule your appointment, please call us at least one week prior to your visit.
PREPARE FOR YOUR APPOINTMENT
- If you are having lab work done before your appointment, fast for 12 hours (no food or drink except water) prior to your blood draw.
- Complete and return the food frequency questionnaire 7-10 days before your visit. Our dietitian will have a full analysis ready at your visit.
- Bring your completed cardiovascular clinic patient questionnaire.
- Bring results of any requested laboratory tests.
- Mail or bring any medical or personal records related to previous hospitalizations, cardiovascular risk factor values and/or treatment.
Plan on being at the clinic for one to two hours.
AFTER YOUR APPOINTMENT
- To reorder a prescription, have your pharmacy call 650-723-6459 or fax a refill request to 650-724-1444.
- If you have questions, call us and ask to speak to our clinical nurse specialist and coordinator, Mary Ann Champagne.
