The Stanford Medicine Thoracic Cancer Program in Pleasanton brings the benefits of a nationally renowned medical program to an East Bay address. We are a multidisciplinary team that prioritizes quality and safety, and comprehensively treats both thoracic cancers and benign conditions, including lung cancers, esophageal cancers, and hiatal hernias, as well as mediastinal tumors, emphysema, and hyperhidrosis.

Please fax the Medical Record Release Form to your new patient coordinator. The medical release form is an authorization form for external facilities to release medical records to Stanford Health Care.

- Recent/relevant typed clinical notes/test results (history and physical, MRI/CT/X-ray results)
- Proof of insurance
- Authorization information (if required)
- Send referrals online
- Place radiology orders
- View referral status
- Access medical records
