Continuing Care Network (CCN)

When you are medically ready to leave the hospital, you may require additional care after you are discharged. At Stanford Health Care we want to provide the best opportunity for you to recover. That’s why at Stanford Health Care we developed the Continuing Care Network (CCN).

 Although we understand you have the choice to choose where you would like to go to receive your post-acute care, we want to make you aware of a few options we encourage you to consider.

The CCN is a group of Skilled Nursing Facilities (SNF) and Home Health Agencies (HHA) who are committed to ensuring a smoother transition. These facilities provide high quality care and clinical rehabilitation services, and adhere to a high level of CMS/regulatory requirements to continually raise the quality of care provided to patients.

We select the organizations for the CCN after reviewing data from the Centers for Medicare & Medicaid Services (CMS) who reviews the ratings of these providers. To learn more about CMS rankings, go to cms.gov or view the CMS Care Compare tool.

Our CCN network includes:

• Skilled Nursing Facilities (SNF): Offering medical care and targeted rehabilitation.
• Home Health Agencies (HHA): Providers visit your home to help with improving your functional skills including balance, walking, and activities of daily living.

Some of the SNF’s in our network have Stanford Health Care physicians overseeing the medical management of patients who have transferred from Stanford Health Care and who have more complex clinical needs. These SNFs are listed under the Provider Care Network in the Skilled Nursing Facility guide.
