Comprehensive Pain Management

The Pain Management Center at Stanford University Medical Center offers a comprehensive range of services for patients with acute or chronic pain.  Through the combined expertise of a multidisciplinary team of pain specialists, Stanford offers the most advanced treatment options currently available, in a supportive, compassionate environment.

Your physician may refer you to the Pain Management Center for evaluation, for one or more specific procedures, or for ongoing treatment.  We have prepared this information to provide an overview of services available at the Center and to explain what you may expect during your treatment.

MEDICAL RELEASE

Please fax the Medical Record Release Form to your new patient coordinator. The medical release form is an authorization form for external facilities to release medical records to Stanford Health Care.
