Inpatient Diabetes Care
Our inpatient Diabetes Care Team is available if you are in the hospital and have diabetes and glucose problems during your stay. We provide inpatient education and discharge services that help you transition to managing your condition at home and support your continuity of care.
As an academic medical center, we get firsthand experience with new technology and innovations in diabetes care. Our providers are at the forefront of diabetes prevention, management, and treatment.
Our highly experienced staff works together to determine the most effective treatment plan for you. Because diabetes is a lifelong disease, our Certified Diabetes Educators®, endocrinologists, and other specialists will provide continuous, supportive care to help you manage the condition to achieve optimal health. INNOVATION HIGHLIGHT
Stanford successfully introduced the use of hybrid closed-loop insulin delivery systems in children ages 7 to 14 with type 1 diabetes. The system adjusts insulin levels based on automatic sensor readings. This advanced technology improves glucose control and significantly decreases the risk for overnight seizures and other long-term complications linked with type 1 diabetes.
The Stanford Health Care Diabetes Education and Prevention Program is certified by the American Diabetes Association for quality self-management education and support. We offer a variety of ongoing classes and workshops in English to help you manage your diabetes and attain your health goals. You may also request individual education sessions for diabetes self-management training. We also offer individual appointments in both English and Spanish. Diabetes Prevention Program
We have a program nationally accredited by the Centers of Disease Control and Prevention for diabetes prevention. This online program is available to Stanford Health Care patients who have pre-diabetes or other risk factors for type 2 diabetes. It is available through a referral from your primary care provider.
This highly successful program is offered over the course of a year through 22 sessions. The program offers:
- Diabetes prevention through coaching to establish sustained behavior change.
- Metabolism tracking.
- Technology that syncs with your MyHealth app to track your fitness, weight, and diet.
Diabetes Education Online Class Information
Due to the impact of COVID-19, Stanford Health Care has taken extra measures to protect patients and staff. We have converted all diabetes education sessions to online video visits.
The Diabetes Care team works collaboratively with you and your providers to help you meet your health goals. The following comprehensive individual and group education programs are available by referral only. Please talk with your provider or contact the Diabetes Care Program to learn more.
Pre-Diabetes: Take Control: Pre-diabetes is a condition in which blood glucose levels are higher than normal, but not yet high enough to be diagnosed as diabetes. Individuals with pre-diabetes are at risk of developing true diabetes. Taught by a dietitian, this course will help you develop personalized strategies for diabetes prevention.
Core Concepts of Diabetes Management: Explore strategies that promote well-being and decrease the risk of complications associated with diabetes. This program focuses on the tools and techniques needed for diabetes management. The course is delivered as a two-part series taught by a diabetes educator, a dietitian, and a pharmacist.
Carbohydrate Counting: The Basics: Learn how different foods affect your blood glucose levels. Discover how to eat the foods you enjoy while still maintaining your goals. This two-hour program is taught by a registered dietitian who specializes in diabetes education.

For your first appointment, you should plan to bring any related test results, your medical history, and any diabetes devices to share with your health care team. Please bring along a completed diabetes self-assessment (English) and (Spanish). Our clinic receptionists will confirm the any additional information you should bring with you prior to your first appointment.
You will also need to have the following information with you when you check in:

- A form of payment for any co-pays or deductibles
- One form of photo ID:
- Valid state-issued driver’s license

Please print, fill out, and return the Medical Record Release Form to your new patient coordinator. The medical release form is an authorization form for external facilities to release medical records to Stanford Health Care.
Always feel free to bring someone with you to your appointments. A family member or friend can help ask questions, remember the information your care team gives you, and provide support.
Write down your questions before your appointment and rank them in order of importance, beginning with the most important ones. If there isn’t enough time to have all your questions answered during your appointment, ask your doctor who you can speak with to get your other questions answered.

To determine if a clinical trial is right for you, talk to your doctor. He or she can refer you to a research coordinator for more information on studies that may be right for your specific condition.
You can also find the guidelines for who can participate in a particular clinical trial online. However, it is best to work with your doctor to decide the right care approach for your needs.

Prevention & Prediabetes
If you have prediabetes, elevated blood glucose (high blood sugar) levels place you at risk for developing Type 2 diabetes.
Our registered dietitians and certified diabetes educators work with you to custom-tailor a lifestyle plan to help you successfully lower your diabetes risk and reverse your prediabetes.
Your diabetes prevention plan may include:

Our diabetes specialists work together to customize a lifestyle plan just for you. We use sophisticated technology to monitor blood glucose levels remotely regardless of your location. With this information, we can quickly help you make appropriate changes to stay well.
Type 1 diabetes occurs when the body doesn’t produce insulin. This hormone moves glucose (blood sugar) from the bloodstream into cells for the body to use. Without insulin, glucose builds up in the blood. High glucose levels can be dangerous, leading to heart and kidney disease, nerve damage, and vision problems.

Type 2 Diabetes
People with Type 2 diabetes produce insulin, but the hormone doesn’t move glucose (blood sugar) from the bloodstream into cells the way that it should. As a result, blood glucose levels rise. Over time, elevated glucose levels can damage the heart, kidneys, nervous system, and eyes. If you have Type 2 diabetes, it’s important to monitor blood glucose and take medications as prescribed.

Our pharmacists will help you manage any medications you may need to take. They will ensure that any drugs you take to manage other conditions will not interact with your blood sugar levels or your diabetes medications. They will also show you how to use an insulin pen, pump, or glucose sensor.
