Stanford Health Care’s Community Health & Partnerships has been an early supporter of Destination Home's countywide Homelessness Prevention System. This initiative represents a distinctive collaboration among public agencies, private funders, and non-profit organizations aimed at assisting at-risk families and individuals in maintaining their housing and preventing homelessness.

The recent publication of a national toolkit, developed in partnership with Results for America and the Wilson Sheehan Lab for Economic Opportunities at Notre Dame, serves as an essential resource that builds upon the success of the local Homelessness Prevention System in Santa Clara County. This initiative has demonstrated its effectiveness by supporting 17,000 at-risk residents in remaining housed—93% of whom continued to reside in their homes two years later—resulting in stability for families, with every $1 invested in prevention yielding $2.47 in public benefits.

Rigorous research has substantiated this prevention model, confirming its efficacy in preventing homelessness rather than merely postponing it.
