Below is a selection of On-line Classes that have been developed specifically for patients and their Families who have Diabetes in the Transplant Process through the Stanford Hospital and Clinics Transplant Diabetes Program. PREPARE FOR YOUR APPOINTMENT

During the transplant process, your blood glucose control and nutritional needs may change.

In anticipation of these changes, we have made available a Blood Sugar Log and Food Diary Log that you or your family can download, complete and bring to your appointment so we can monitor your progress while at home.

The information you provide will help us better accommodate your needs. Please feel free to write down any questions and or concerns you may have before your appointment.
