Stanford Health Care accepts most major commercial health plans that employers offer their employees.
Some employers choose to offer “narrow” networks that have limited options in their network. You should always check with your employer’s benefit department or your health plan to determine if there are any unique or narrow networks within your coverage.
Stanford Health Care accepts the following employer-sponsored group plans.
If you have a HMO plan: Stanford Health Care is in-network for certain HMO plans. However, these HMO plans are for Stanford specialty services only, not for Primary Care Physicians (PCPs.) To choose a PCP, please check your health plan’s website or contact your health plan directly.
To be seen at Stanford Health Care, your PCP or specialist must refer you and your medical group or health plan must authorize all services provided at Stanford Health Care or by Stanford Health Care physicians.
If you have a PPO/EPO/POS plan: Stanford Health Care is in-network for hospital and physician services. This includes specialist and PCP services.
If you have a HMO plan: Stanford Health Care is in-network for certain HMO plans. However, these HMO plans are for Stanford specialty services only, not for Primary Care Physicians (PCPs.) To choose a PCP, please check your health plan’s website or contact your health plan directly.

Anthem offers some plans with a narrow network and in such cases, limits the providers that are allowed to participate in the product/network.
Whether your plan is HMO, PPO/EPO or POS, for the most accurate information and to verify if Stanford Health Care is in network with your specific plan, please call the Anthem member services department (phone number is on the back of your ID card). When calling Anthem, it may be helpful to reference Stanford Health Care’s Tax ID number in your verification process. Stanford Health Care Physicians and Hospital Tax ID is: 77-0465765

Stanford Health Care and its physicians are in-network with out of state Blue Cross/Blue Shield plans, as long as the plan uses the nationwide network of providers. This is known as the Blue Card network. Most employer sponsored group plans will include this network, especially if they have employees located in multiple states. Please contact your health plan or your employer to inquire about access to this network.

Kaiser members must obtain a referral from their Kaiser Primary Care Physician (PCP) or specialist and an authorization from their health plan for any services to be covered and considered in-network at Stanford Health Care. You would not be able to select a Stanford Health Care physician as your PCP.

If you have a PPO/EPO/POS plan: Stanford Health Care is in-network for certain PPO/EPO/POS for hospital and physician services, including specialty and PCP services. United Health Care offers plans with a narrow network and UHC limits the providers that may participate in the product. The CORE product is an example of a PPO plan that has a narrow network - Stanford hospital is considered in-network and Stanford physician services are considered out-of-network. Refer to your employer or United Health Care to determine if your plan has a narrow network of providers.
