About the Stanford Neuroscience Health Center
Built from the ground up in partnership with our patients, the Stanford Neuroscience Health Center is a one-stop care destination designed to meet the unique needs of people with neurological disorders.
The Stanford Neuroscience Health Center features a streamlined patient check-in and registration system that allows you to schedule several appointments during one visit and move from floor to floor without additional check-ins.
Each area of the Center provides amenities for specific patient conditions to support comfort, minimize sensitivities, and ease mobility difficulties.
For Patients
For your convenience, you may check in for all same-day appointments at the Stanford Neuroscience Health Center through a centralized, check-in desk near the front lobby. In addition to all outpatient services, you also can access onsite pre-surgery consultations at the center.
MEDICAL RELEASE
Please fax the Medical Record Release Form to your new patient coordinator. The medical release form is an authorization form for external facilities to release medical records to Stanford Health Care.
