The Radiation Oncology Clinic in San Jose at Cancer Center South Bay offers the latest radiation therapy treatments and technology, including accelerated partial breast irradiation (APBI) options for women with early-stage breast cancer. Please fax the Medical Record Release Form to your new patient coordinator. The medical release form is an authorization form for external facilities to release medical records to Stanford Health Care.
