Addressing Community Health Needs
Every three years, Stanford Health Care conducts a community health needs assessment in partnership with local public health departments, community health experts, and residents, discovering ways to better serve our most vulnerable and underserved communities.

OUR FOCUS
The 2022 Community Health Needs Assessment (CHNA) identifies pressing health disparities, uncovers gaps in care, and offers strategies to address the drivers of and barriers to poor health across Santa Clara and San Mateo counties. These are the factors that patients and their families bring with them when seeking care at Stanford Health Care and influence their ability to get healthy, stay healthy, and be healthy at home.
Based on community priority and severity of disparities, the 2023 – 2025 Implementation Strategy Report reflects Stanford Health Care’s plan for addressing the following community health needs:
