Your dedicated International Medical Services team member is a vital point of contact to help manage your entire hospital experience—from initial inquiry to treatment to follow-up care. Our staff will conduct a comprehensive orientation to discuss medical and financial arrangements and introduce you to the resources available at Stanford Health Care and Lucile Packard Children's Hospital.
To work with us, please review and complete two important forms—International Patient Financial Policy Form and Patient Information Form (Adult or Pediatric)—and send these to us via email or fax before booking your appointment. These forms will enable us to pair you with the best team member to help with your specific situation.
Step 1: Patient Intake Forms (Adult and Pediatric)
After submitting the Patient Information Form via fax or email, we will provide you with courier instructions so that you can send any relevant medical records, physician reports (in English), and films (X-ray, MRI, CT scan, ultrasound), if needed.
Step 3: Appointment Confirmation
Once the Patient Information Form and your medical information are received, your Patient Coordinator will confirm the date and time of your appointment.
Step 4: Visa Assistance
Once your Patient Coordinator has booked your appointment, a verification letter can be provided if needed.
