Medical Staff: Credentialing and Privileging
Annual Physician Education/COI/Confidentiality
To complete the education requirements:
- Annually practitioners are required to review a select set of materials essential to maintaining compliance with regulatory standards, including those set by The Joint Commission, California Title 22, and CMS for each entity providers are credentialed.
- Review the CME statements and disclosure information here.
- Follow the Entity headings on the website to guide you through the review process. Clinicians who are on staff with multiple Entities are required to review the education for each Entity where credentialed.
- When all education materials have been reviewed, use the link provided here or click NEXT at the bottom of the page to verify that you have reviewed the appropriate documentation. Once submitted, your records will be updated to reflect the completion of your education requirements: Submit Verification Form.
