Reappointments and Privileging: What You Need to Know

All credentialed and / or privileged practitioners have access to a personalized, password-protected Practitioner Home Page (PHP) from which they may complete credentialing and privileging tasks.  Practitioner Home Page information 

If you are a new medical staff or APP applicant and submitting your credentialing application for the first time, please visit the new applicants page for futher instruction.

Reappointment cycle

Appointments are typically valid for two years (24 months).

You will receive e-mail(s) notifying you of reappointment tasks and PHP account information approximately four (4) months prior to your reappointment application deadline. In order to ensure timely processing, please complete your reappointment application promptly.

You are always welcome to contact your credentialing coordinator if you have questions regarding your reappointment or need assistance accessing your Practitioner Home Page.

Once you have submitted your application, your PHP will be updated to reflect the status of your reappointment application.

Privileging requests

You may view your current clinical privileges as well as request additional privileges from your Practitioner Home Page (PHP). Accessing your PHP.

If you are enrolled in a Stanford Health Care (SHC) fellowship program, privileges will only be granted in areas outside the fellowship that meet appropriate criteria.
