Stanford Health Care now provides quality primary care for home-bound older adults. Our specialists come to your home for routine primary care needs — including same-day or next-day home visits for new medical issues that may arise.

What We Offer You for Home-Based Senior Care
- Expertise in primary care of older adults who have impairments in mobility or memory that make it difficult to leave home. Go to Primary Care
- A team-based approach to address all of your health needs. Your interdisciplinary team will include a geriatric medicine-trained nurse practitioner, doctor and social workers. We also help coordinate with specialists and community services. Go to Your Care Team
- Comprehensive support services to help you, your family and caregivers better manage health care needs as you age. Go to Support Services
- Ease of access and convenience of providing care in the environment where you are most comfortable — your home. We can provide same-day, or next-day home visits for new medical conditions that may arise. Go to Enrolling in Care
Through our Stanford Home-Based Senior Care service, we provide compassionate care and support for older adults. Our interdisciplinary team of geriatric medicine specialists can treat a range of primary care needs in your home — eliminating the burden of having to travel for routine care. Aimed at homebound older adults, this transfer of primary care is not for one-time or time-limited home visits.
The at-home care services we offer include:
- Primary care for older adults who may have neurologic conditions like dementia, cognitive impairment, or other neuro-degenerative diseases
- At-home exams for routine and urgent visits.
- Caregiver support and resources
- Fast referral and coordination with Stanford Health Care
- Help with prescriptions and refills

Our supervising geriatrician specializes in the treatment of dementia, geriatric education, and home care medicine, and in improving the care of older adults with cognitive impairment.

Our geriatric nurse practitioner and physician assistant have geriatric medicine training and under the guidance of the team physician, will manage your care including appointment coordination, sharing your test results and providing treatment. They also provide support and education, so you and your family fully understand your care plan.

Nothing should stand in the way of getting good health care – particularly as you age. We offer quality care and convenience of home-based primary care to existing Stanford Health Care patients who meet the following requirements:
- Age 80+, or 65+ with a diagnosis of dementia
- Needs total assistance with 2 activities of daily living (such as dressing, bathing, transferring, or eating)
- Hospitalized at Stanford within the last year or had two emergency room visits to Stanford within the last year.
- Resident of San Mateo and Santa Clara County, within our service area*
- May be living at home, in an assisted living facility, board and care home, or memory unit. We are not able to travel to skilled nursing facilities.
